Case management services to their clients
✨How do we achieve this❓
➡️ On receipt of the case management instruction from a solicitor or insurance company, we meet with the individual and their family/caregivers to gain an in-depth understanding of the impact of the injury on the individual’s:
🔸physical
🔸emotional
🔸psychological function
➡️We gain an understanding of how the injury affects their ability to undertake their normal daily activities including:
🔹self care
🔹education/work
🔹leisure
➡️Through discussion with the individual and their family, we identify the immediate needs of the individual including:
✅ rehabilitation
✅ care
✅housing
✅equipment
✅transport
✅work/education
➡️We source the appropriate support for the individual either from statutory services if timeframes are acceptable, or private suppliers if the waiting lists are protracted for statutory services.
➡️Our goal is to ensure the individual receives cost effective intervention from the right people at the right time 💫
➡️We support the individual through their rehabilitation journey, reviewing and evaluating services in place at regular intervals to ensure the individual’s needs are met at all times.
